What is the manufacturer and model number of this bent wire fixture piece? I have scoured the internet and cannot find it, or even another image of one.

I do not know. I purchased it from our local hardware store. It was a fixture they use on their own displays. I asked them if they had one I could buy since it was perfect for my limited space. They thankfully located an extra in the back and were willing to sell it to me. Sorry, I could not be more helpful.
